Contract Submission

We are currently experiencing a high volume of applications; processing times may be longer than the 14-business day standard

Please use this form to submit documentation that requires approval from the ON Provincial office. 

  • Contract or agreement with a location such as a community centre or church for any unit activity such as:
    • sleepover
    • cookie selling
    • unit camp
    • special event
  • Regular unit meeting space (except for school board permits)

The applicant is responsible for the following:

  • Completing the agreement/contract (except for signature)
    • PLEASE NOTE: Agreements/contracts that do not have all the relevant information included will be sent back to the submitter and will need to be resubmitted for signature.
  • Payment arrangement
    • Regular unit meeting space – payment will be made on the unit’s behalf to the facility, please ensure any payment information is included with the submission as fees do need to be approved by Ontario Council.
    • Events or other activities - the submitter is responsible for making payment to the facilities.
  • Requesting insurance if needed, INS.02
    • It is the responsibility of the individual submitting the agreement/contract to provide the facilities with proof of insurance – please reference link above for details on how to obtain a certificate of insurance.

The office will:

  • Arrange for signature.
  • Payment if applicable (regular unit meeting space only)
  • Return signed contract to applicant.

*Please allow up to 14 business days for processing. If your request is due in less than 7 business days, we will do our best to fulfill your request, but we cannot guarantee that it will be processed within your required timeline.
